Students attending their first year in public school are required to have a dental check-up by May 31st of his/her first year.  Download the form hereand bring to your dentist for completion.

California law requires a health examination on school entry.  Download the form hereand bring it to your child's physician for completion.  Alternatively, you may sign the health exam waiver found here

Please complete the forms and return them to the office along with a copy of your child's birth certificate and record of immunizations.
